Forms and additional information for students seeking assessments

Students who believe they may need an assessment should contact their Accessibility Advisor at their college/university, who will determine if an assessment is needed. Accessibility Advisors can refer a student to RARC by emailing rarc@queensu.ca to start the documentation collection process. 

Please select the appropriate referral package from the drop-down below.  All RARC referrals (whether conducted on-site at RARC or by a MAT practitioner) use the same referral packages. 

All personal documents being sent to RARC should be password protected or sent via a secure format such as through SecureDocs . Documents should be sent to rarc@queensu.ca.
